Diploma Information Confirmation

Google Forms

The Diploma Information Confirmation Google form was developed out of a need to find a faster, more efficient way to validate and update information of record in the Registrar's Office. This artifact supports ISTE Standard 3.a and 3.c for Teachers as it demonstrates the transfer of knowledge to new technologies and situations. The previous process for collecting the information gathered through the Google form was paper intensive and extremely slow, requiring several weeks in order to ensure the minimal amount of errors. By adapting the multi-step Google form to fit this process we have reduced the time it takes to collect and correct the relevant information. By using a mail merge process in conjunction with the inclusion of current information of record (i.e. Diploma Name and Diploma Address) I was also able to provide students with the information we currently had on record and through the Google form allow them to confirm or update the information on record. This reduced the number of errors on diplomas and addresses, reduced the cost of mailing diplomas (less returned mail), and with the further addition of tracking numbers (also provided to students via email) we have seen a significant reduction in the number of diplomas lost by the postal services. This last was more significant when mailing diplomas internationally.
